Landelijk Schakelpunt (LSP - or National Exchange Point)

Our general practice is connected to the Landelijk Schakelpunt LSP (National Exchange Point). The LSP allows healthcare providers to exchange information about your health via the internet through a secure connection. The goal is to be as well-informed as possible about your health in relevant situations so that the most appropriate care can be provided to you.

Through the LSP, information can be exchanged between your GP, the out-of-hours GP service (HAP), your pharmacy, and your treating medical specialists. This involves a portion of the data from your GP record and your medication record.

Consent

To exchange your data via the LSP, you must give your consent. If you want us to share information about you via the LSP, you can arrange your consent in the following ways:

  • Online:
You can give your consent via the website ‘Volg je zorg’.
  • Orally:
You can inform your GP and pharmacy.
  • In writing:
You can download, print, fill out, and give this consent form your GP and pharmacy

Before you give your permission, please read the leaflet 'Yes! I want to share my medical records'.

Of course, if you have any questions, you can also contact our practice.
