If you want to register with our GP practice, you can use the registration form. After we have received the completed form, we will contact you by telephone to schedule an introductory meeting. We think it is important that you have a good relationship with your GP before we register you as a patient. After the introductory meeting with the GP, we can convert your registrion into being a patient at the practice
Our working area for new patients is Bussum. Within this work area, we can be with you within fifteen minutes in an emergency and we can guarantee good care. If you are already registered with a GP practice in this area, we request that you first contact us by telephone.
If you change of GP practice, you must inform your previous GP practice of your departure and submit a request to forward your file to our GP practice.
We are obliged to check your identity on your first visit to our GP practice, therefore we ask you to then bring a valid ID.
